SMS Sponsorship Program Application

By becoming part of the SMS program, you agree to allow SMS to use your name, likeness, and photographs for all advertising, website promotion and sales. You are agreeing to purchase all your race related products and fuel during the 2006 season from SMS exclusively, with exception to items that SMS does not carry or have access to. SMS stickers will be provided to you to run on each side of your top wing, or body if running a full bodied car or non-wing car. Failure to adhere to all terms of the sponsorship agreement will negate you from receiving sponsorship benefits and you may be responsible for paying back the difference between retail price and sponsorship prices. In exchange, SMS will offer you a 5% discount on all products currently carried at the time of this printing (future products may be included or excluded from discount) SMS will also offer you a $.10 per gallon discount on all racing fuels. Please complete the following information, and by signing you acknowledge and agree to all terms of this contract.
